I/We the undersigned will not hold the W.R.A.C. Society responsible for any accidents to myself or any member of my family, or to guests while proceeding to, during, or returning from any club sponsored event. I/We also understand that membership may be forfeited if library privileges, stated in club policies, are not adhered to.

(Provides club privileges for up to 2 persons per household)
